Description

The purpose of this program is to establish or expand Assertive Community Treatment (ACT) programs for transition-aged youth (age 18-25), adults, and/or older adults with a serious mental illness. ACT is an intensive, team-based, community-based service delivery model that provides comprehensive, multidisciplinary care, including 24/7 crisis response, medication management, therapy, case management, and support with housing, employment, and daily living needs.

Eligibility

Eligibility is statutorily limited to States/Territories; political subdivisions of a State, Indian Tribe or Tribal organization (as such terms are defined in section 4 of the Indian Self-Determination and Education Assistance Act); mental health system; health care facility.
